5. How to request a record

  1. Complete the prescribed request form (Form 2 of the PAIA Regulations), available from the Information Regulator's website or from us on request.
  2. Send it to the Information Officer at legal@innovationconsult.co.za or deliver it to our address.
  3. Give enough detail to identify the record, say how you want access, and state the right you want to exercise or protect and why the record is needed for it.
  4. Pay the request fee and any access fee prescribed by the PAIA Regulations. We will tell you if a deposit or fee is payable.

We will decide on your request within 30 days, or tell you if we need to extend that period as PAIA allows. We may refuse access on the grounds set out in Chapter 4 of Part 3 of PAIA, for example to protect another person's privacy, commercial information or confidential information. If we refuse, we will give reasons and tell you how to complain to the Information Regulator or apply to court.
